WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK & INDEMNIFICATION AGREEMENT

READ CAREFULLY BEFORE PROCEEDING TO PAYMENT. THIS AFFECTS YOUR LEGAL RIGHTS.

In consideration of being allowed to participate in the Turkey Thanksgiving Shootout (the “Event”) hosted by Richmond Floorball Club (RFC), I (“Participant”) agree as follows:

  1. Assumption of Risk
    I understand that floorball and related activities involve inherent risks, including but not limited to: collisions with other players, sticks, balls, boards, goals, rink/floor surfaces; slips/falls; cuts; bruises; sprains/strains; broken bones; dental/eye injuries; concussion or other head/brain injury; heat illness; cardiac events; and in rare cases permanent disability or death. I voluntarily and knowingly assume all risks of participation, whether known or unknown, foreseeable or unforeseeable, and regardless of cause.

  2. Release and Waiver of Liability
    To the fullest extent permitted by law, I waive, release, and forever discharge RFC and all related parties — including officers, directors, volunteers, staff, referees, contractors, sponsors, and the owners/operators of River City Sports & Social Club Annex (collectively, the “Released Parties”) — from any and all claims, demands, causes of action, damages, losses, or liabilities of any kind arising out of or related to my participation in the Event, including travel to and from the Event, except for claims that cannot legally be waived.

  3. Medical Fitness & Responsibility
    I certify that I am physically and medically able to participate. I have no condition that would make participation unsafe. I will stop participating if I feel unwell or injured. I accept full responsibility for any injury or illness I sustain.

  4. Emergency Medical Authorization
    If I am injured or become ill during the Event, I authorize RFC staff/volunteers to obtain emergency medical care for me. I understand I am solely responsible for all costs of such care. I authorize the sharing of relevant medical information with emergency responders if needed.

  5. Rules, Safety, and Conduct
    I agree to follow all Event rules, IFF/USA Floorball rules as applied, venue policies, and staff/referee instructions. I understand that unsafe, disrespectful, or unsportsmanlike conduct may result in removal from the Event without refund, and that removal does not create liability for the Released Parties.

  6. Indemnification / Hold Harmless
    To the fullest extent permitted by law, I agree to defend, indemnify, and hold harmless the Released Parties from any claim, suit, cost, or liability brought by or on behalf of me or any third party arising from my participation in the Event or my actions during the Event.

  7. Communicable Disease Acknowledgment
    I understand participation may increase exposure to communicable diseases (including but not limited to COVID-19 and influenza). I voluntarily assume this risk and agree to comply with any posted health/safety guidance.

  8. Photo / Video / Media Release
    I grant RFC permission to photograph, film, or otherwise capture my likeness during the Event and to use such media for RFC/Atlantic floorball promotional, educational, or reporting purposes in any medium, without compensation. I waive any right to inspect or approve final materials.

  9. Personal Property
    I am responsible for my own equipment and belongings. The Released Parties are not liable for lost, stolen, or damaged property.

  10. Severability
    If any portion of this agreement is found invalid or unenforceable, the remaining portions shall remain in full force and effect.

  11. Governing Law / Venue
    This agreement is governed by Virginia law. Any dispute arising from this agreement or the Event shall be brought in the appropriate courts of Virginia.
